I have read the contract and operations manual, and I understand all of the rules and guidelines stated in the document. I agree to follow all rules, and I understand that failure to follow the rules may lead to suspension or cancellation of the contract.
Acknowledgment of Risk and Release of Liability
I understand that participation in activities at this facility, including but not limited to the use of bats, balls, and other sports equipment, involves inherent risks that can result in serious injury or death. By signing below, I acknowledge that I am voluntarily assuming all such risks for myself and/or any minors under my care, and I agree to release and hold harmless the facility, its owners, staff, and affiliates from any and all liability arising from participation in activities or use of the premises.
